As a Physical Therapy Assistant (PTA) for Home Care you will provide compassionate, high-quality physical therapy services to patients in their homes while working under the direction and supervision of a licensed Physical Therapist. The PTA will implement individualized treatment plans, monitor patient progress, and help patients improve mobility, strength, and independence. This role requires excellent clinical skills, strong communication, and a commitment to delivering exceptional home care services.
Qualifications
- Graduate of an accredited Physical Therapist Assistant (PTA) program.
- Current Minnesota Physical Therapist Assistant license in good standing.
- Knowledge of physical therapy principles and rehabilitation techniques.
- Maintain required continuing education to keep licensure current.
- Compassionate, patient-centered approach with a genuine interest in helping individuals achieve their rehabilitation goals.
- Strong communication, interpersonal, organizational, and time management skills.
- Ability to work collaboratively with patients, families, and the interdisciplinary healthcare team.
- Commitment to maintaining confidentiality and following organizational policies and procedures.
Responsibilities
- Provide quality physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
- Implement individualized treatment plans using therapeutic exercises, physical agents, assistive devices, and other rehabilitation techniques to improve strength, mobility, function, and independence.
- Prepare patients, treatment areas, and equipment while maintaining dignity, privacy, and safety.
- Monitor patient progress, respond to changes in condition, and communicate updates to the Physical Therapist and healthcare team.
- Educate patients, families, caregivers, and staff on treatment techniques, functional activities, and safe mobility practices.
- Assist with evaluations, data collection, documentation, care planning, and insurance requirements as directed.
- Maintain accurate and timely clinical documentation and billing records.
- Collaborate with the interdisciplinary team, participate in discharge planning, and provide recommendations to support patient goals.
- Maintain department standards, equipment, and organization while following policies, procedures, and productivity expectations.
- Provide consultation, training, and support to other departments as requested.
- Perform other duties as assigned.
