Job DetailsJob Location: Breckenridge, MN 56520Salary Range: $27.15 - $37.58 HourlyPosition Summary This position is responsible for delivering community-based services to adults with mental health diagnoses.  This role provides support in areas including mental health and emotional wellness, life management skills, emotional regulation, budgeting assistance, daily living activities, and homemaking support. Essential Duties and Responsibilities % Assists families and individuals in life management skills, such as developing community resource mobilization, transportation needs, and other family community support services. Documents and reports unusual events, statements and/or occurrences observed or discussed with individuals and inform case manager and/or supervisor of these events. Assists families and individuals in obtaining and maintaining living arrangements that are least restrictive and most appropriate for the individual and/or child/children. Provides intensive training in life education, emotional regulation skills, daily living skills, basic parenting skills, home management skills, dealing with chronic health issues (e.g. drug abuse and pregnancy), and advocacy to individuals and families. Prepares social outings and group social skill activities to model and reinforce appropriate behaviors. Provides crisis intervention to families as needed for medical and mental health crises. Provides ongoing support services to child protection and child welfare clients, including supervised visitation, client transport, and Parent Support Outreach Program case monitoring. 35 Works closely with and informs the case managers (social worker), mental health practitioners, clinical supervisors, treatment director and/or social services supervisor of any changes in individual status, symptoms, physical health or other behavioral issues or situations that may lead to abuse or neglect of self or others. Provides ongoing support services to child protection and child support clients, as directed. Attends and participates in required meetings with children's case manager, mental health practitioners, mental health professionals, clinical supervisor, treatment director, social services supervisor, clients, legal representatives and others to meet program requirements. 20 Documents and reports unusual events, statements and/or occurrences observed or discussed with families and/or child/children and inform children's case managers and/or supervisor of these events. Documents information in Social Services Information System case files and prepares written reports per agency standards and timelines. 20 Assist families in obtaining and maintaining living arrangements that are least restrictive and most appropriate for the child/children. 5 Give testimony in court for agency and/or may accompany child/children to court and provide support. 5 Performs other duties of a comparable level/type, as required. N/A QualificationsQualifications Education: Associate’s Degree in Occupational Therapy, Liberal Art, Human Services or related field and at least two years of related experience. Requirements: Valid MN Driver's License.     Physical Demands and Working Conditions Regularly read and interpret documents and other written material. Regularly communicate and exchange information with others in person, over the telephone, and using written communication methods. Occasionally move or transport items weighing up to 20 pounds and occasionally lift and/or move up to 50 pounds. Frequently exposed to adverse working conditions such as environmental issues (challenging behaviors, imminent danger, threatening environment) or poor atmospheric conditions. Moderately exposed to hazardous materials (chemicals, blood and other body fluids, etc.) Seldom exposed to intense noise or extreme temperatures. Frequent travel is required. Equipment Utilized Automobile, office equipment, computer, and software.
