Kitsap Mental Health Services (KMHS) is seeking a Discharge Coordinator to support clients transitioning from residential treatment at the Pacific Hope and Recovery Center (PHRC). This role helps ensure clients leave treatment with the connections and resources needed to continue their recovery. The work is centered on coordination, problem solving, and helping individuals navigate systems that can be difficult to access during times of crisis. Primary Responsibilities:

• Coordinate and implement timely discharge plans for clients supported at PHRC.
• Collaborate with SUDPs, BHCs, and MHPs to develop discharge plans that address complex, multi-system needs and barriers.
• Support case management tasks related to discharge planning, including coordination of mental health, psychiatric, medical, and substance use services.
• Assist with housing referrals, transportation supports, DSHS-related services, outpatient medical and dental connections, and follow-up appointments.
• Participate in daily staff meetings and stabilization planning reviews.
• Facilitate approved group activities focused on community resources and post-discharge support.
• Provide transportation to appointments or community resources as needed.
• Maintain effective communication with unit staff, care teams, and external partners involved in discharge planning.
• Complete all required electronic and paper documentation in accordance with agency policies and confidentiality standards.
• In the absence or shortage of Behavioral Health Technicians, assist with unit support tasks such as safety checks, intake and discharge processes, referral coordination, and meal preparation.

Minimum Qualifications:

EDUCATION: High School Diploma or GED

EXPERIENCE: Entry level (minimum 1 year of job-related experience) Valid Washington Food Handler card.

This position requires driving clients in a personal vehicle on behalf of the agency. Incumbent must meet all agency driving requirements, including being at least 21 years old, having been licensed for a minimum of three years, successfully completing a motor vehicle history check, possessing and maintaining a current, valid driver’s license in the state of Washington, and having reliable, insured transportation.

LICENSURE: Eligibility for Agency Affiliated Counselor Registration

Preferred Qualifications:

EDUCATION: Bachelor’s degree in psychology, social work or a behavioral health field.

EXPERIENCE: Experienced (minimum 2 years of job-related experience). Relevant experience working with clients with severe and persistent mental illness.

KNOWLEDGE:

  1. Maintain a working knowledge of current trends in community mental health, including health care reform and automation of job tasks using computer technology.
  2. Maintain a working knowledge of mental health rehabilitation standards.

SKILLS:

  1. Excellent interpersonal and verbal skills.
  2. Communication - Communicates clearly and concisely.
  3. Computer skills - The ability to operate a personal computer, fax machine, printer, and copier proficiently.

  1. Ability to deescalate potentially volatile situations with clients.
  2. Ability to react quickly and perceive environmental stimuli accurately.
  3. Ability to assist in building positive working relationships with staff of all agency departments

Mental/Physical Requirements: While performing the job duties, the employee is required to walk, sit, use hands and fingers, reach with arms, talk, or listen. Peripheral vision is also required for this position. Frequent mobility and/or sitting required for extended periods.Ā
