What You’ll Do As a Charge Nurse in a skilled nursing facility, you will play an important leadership role in supporting daily clinical operations and ensuring residents receive compassionate, high-quality care. You will help guide nursing staff, monitor changes in resident condition, support care plans, and promote a safe and positive environment for residents and team members. Provide direct nursing care and support to residents in a skilled nursing setting Administer medications and treatments as prescribed Monitor, assess, document, and report changes in resident condition Support and follow resident care plans as directed Complete incident and accident reports as needed Communicate with residents, families, and points of contact regarding status updates or changes in condition Complete narcotic reconciliation with incoming and outgoing shifts Provide leadership and support to CNAs and other nursing team members Follow facility policies, procedures, and regulatory standards Assist with other duties or projects assigned by the Director of Nursing or Administrator What We’re Looking For Must have an active RN license in the state of Texas in good standing Must have graduated from an accredited RN nursing program Skilled nursing, long-term care, or rehab experience preferred Strong communication, documentation, and leadership skills Ability to respond to resident needs and changes in condition with urgency and compassion Must be eligible to work in the United States Why Work for Wellsential Health?
