Position: Nurse Practitioner (NP)

The Nurse Practitioner (NP) provides medical and psychiatric evaluation, treatment, and ongoing clinical care to individuals served across Outreach Development Corporation’s continuum of behavioral health and substance use disorder programs. The NP works collaboratively with the Medical Directors, program leadership, and interdisciplinary treatment teams to ensure high-quality, integrated, and person-centered care.

Job Scope:

  • Conduct psychiatric and medical evaluations for individuals receiving services across Outreach program modalities.
  • Provide initial physical examinations and ongoing medical follow-up as clinically indicated.
  • Prescribe, monitor, and manage medications in accordance with New York State regulations, agency policy, and program licensure requirements.
  • Collaborate with Medical Directors, psychiatrists, and other prescribing providers regarding diagnosis, treatment planning, and medication management.
  • Administer and manage medications as permitted under applicable program licenses and regulatory standards.
  • Communicate effectively with interdisciplinary staff regarding client diagnosis, treatment needs, and the impact of medications on functioning.
  • Participate in case conferences, treatment team meetings, and clinical consultations as appropriate.
  • Maintain accurate and timely clinical documentation, including diagnoses, progress notes, medication records, and treatment plan reviews, within the agency’s electronic health record (EHR).
  • Provide clinical consultation and on-call coverage as assigned to support continuity of care.
  • Ensure compliance with I-STOP, DEA, OASAS, OMH, and other applicable regulatory requirements.
  • Engage in quality improvement initiatives and seek opportunities to enhance healthcare delivery through professional development and external collaboration.

Each role at Outreach has identified production and outcome goals.  These goals may be stated as a number or as an outcome within an identified time frame.  These goals are intended to provide each job holder with the knowledge they need about expected performance.  Accomplishments against these goals will be a discussion point in the annual evaluation.

Supervised by

  • The Medical Director (MD)
  • Administrative supervision by program director or designee

 

Qualifications

  • Must be a licensed Nurse Practitioner and currently registered as such by the New York State Education Department.   
  • Must have at least one year of education, training, and/or experience in alcoholism, substance abuse or chemical dependence services.   
  • Must maintain own malpractice insurance

Work Environment

This job operates in a professional office environment and outside in the field. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Must be able to travel among assigned sites and locations as needed.
