Job DetailsJob Location: Brentwood Mental Health Clinic - Brentwood , NY 11717Position Type: Per DiemEducation Level: Graduate DegreeSalary Range: $85.00 - $91.00 HourlyTravel Percentage: Road WarriorJob Shift: FloaterJob Category: NurseSUMMARY  Nurse Practitioner reports to the Medical Director/Director of Medical Services.  All assigned duties must be within the scope of practice of the supervising physician and appropriate as per the education, training, and experience of the nurse practitioner to provide high quality care to substance use disorder patients.    ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties and responsibilities may be assigned.  Meet the medical assessment and treatment needs of each resident  Provides safety assessment for withdrawal symptoms or other potentially life-threatening behavior or conditions within 24 hours of admissions  Notifies on call physician of immediate crisis  Provide peer to peer reviews to managed care companies and other healthcare entities  Conclude a medical assessment within 7 days of admission and a full history and physical, when necessary, within 21/45 days of admission  Conduct a medical assessment of physical and mental health conditions and medical treatment to stabilize and then enable the resident to manage chronic health and mental health conditions including treatment of physical health conditions that are routine   Documenting in the Medical Record using Welligent, past medical history, physical findings, diagnoses, treatment plan, written orders, and prescriptions if indicated  Ensure timely and accurate clinical documentation to support submission of claims to financial entities   Will provide coverage at Phoenix House locations based on need either in person in person or virtually  Ensure that all necessary follow up, either with outside clinics, Physician, or NP is both ordered and obtained  Responsible to evaluate for follow up or sick call, any patient who has been triaged by a facility nurse  Will provide care and supervise the transfer of patients to the emergency room as warranted    COMPETENCIES (KSAs)  Ethical Practices  Judgment   Relationship Management  Time Management  Empathy  Professional Boundaries  Global and Cultural Awareness  Critical Evaluation  Problem Solving  Decision Making  Communication  Teamwork   Crisis Management  Technical Capacity  QualificationsREQUIRED EDUCATION  Successful completion of an approved nurse practitioner or physician assistant program.  Current approval by the joint Board of Medical Examiners and Board of Nursing to perform medical acts.  Must be currently licensed as a registered nurse in the state.  Because of the nature of responsibilities, it is preferable to have experience in counseling and teaching and have a master's degree.    REQUIRED LICENSES/CERTIFICATIONS  NYS NP License  NPI Number  Medicaid / Medicare Number  DEA / Suboxone Wavier  Current certification in CPR, Infection Control and Child Abuse    REQUIRED EXPERIENCE  Qualified by training, education, and experience in providing medical care and assessment, in collaboration with a physician, to chemically addicted residents and residents withdrawing from alcohol and or drugs  Three Years related experience    WORK ENVIRONMENT  This job operates in an indoor medical clinical environment.  This role also routinely meets patients who may have contagious illnesses.  Appropriate PPE will be provided and required.    PHYSICAL DEMANDS  The physical demands described here are representative of those that must be met by an employee to successfully perform the essential duties and responsibilities of this position.  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential duties and responsibilities.  Regularly required to stand, walk, use hands to type, handle or feel objects, tools, or controls; reach with hands and arms, and talk or hear.   Frequently required to sit, stoop, kneel or crouch and lift and/or move up to 20 pounds.
