Transition Specialist – Full-Time | Vocational Services | Brick, NJ

Schedule: Full-time position, 8:00 AM–4:00 PM, with daytime and community-based hours

Department: Vocational Services Program

Pay Rate: $21.50 per hour, hourly position

As a Transition Specialist in our Vocational Program, you will support adults as they gain hands-on experience, strengthen essential skills, and work toward competitive, integrated employment in their communities.

This position is well-suited for someone who enjoys coaching, teaching, community-based work, and supporting personal development in others.

What You’ll Do

As a Transition Specialist, you will support individuals through structured, goal-oriented activities that build independence, strengthen employability skills, and encourage meaningful community participation.

Skill Development & Individual Support

  • Teach essential life skills, including daily living, social interaction, employability, and problem-solving strategies
  • Facilitate structured lessons and activities designed to support employment readiness
  • Adjust support approaches based on each individual’s strengths, needs, and challenges
  • Promote independence, positive behavior, self-determination, and personal choice

Community Engagement & Learning Opportunities

  • Accompany individuals to community-based sites such as job sampling locations, volunteer opportunities, and inclusive environments
  • Collaborate with community partners and local employers to support meaningful engagement opportunities
  • Provide safe transportation to and from community activities using agency vehicles

Documentation & Individual Goal Monitoring

  • Stay current on each participant’s Individual Service Plan (ISP) and related goals
  • Take part in ISP meetings and contribute recommendations based on observed progress
  • Support goal development and help individuals track progress toward personal and vocational objectives
  • Complete all required documentation, including progress notes, daily logs, medication records, and incident reporting
  • Apply behavior support plans and respond appropriately to behavioral or crisis situations

Health, Safety & Compliance

  • Administer medications in accordance with agency guidelines
  • Follow all individualized plans related to health, diet, behavior, and adaptive equipment
  • Ensure the rights, privacy, dignity, and safety of all individuals are consistently upheld

What We’re Looking For

  • Bachelor’s degree in Human Services or a related field required
  • Valid New Jersey driver’s license in good standing, with no more than 5 points
  • Strong communication skills, both written and verbal
  • Ability to work independently, solve problems effectively, and collaborate with team members
  • Comfortable developing relationships and networking with community partners and employers
  • Must be able to meet physical demands of the role, including lifting 25+ pounds, assisting with mobility needs, and traveling throughout community settings
