This role is a professional RN with service line specific clinical knowledge who offers individualized assistance across the care continuum to patients, families, and caregivers to expedite and coordinate care, and address health system barriers. Assumes responsibility for the coordination of patient care through interdisciplinary and multidisciplinary collaboration to achieve optimal patient outcomes with a focus on high-risk patients or those with complex care needs (including multidisciplinary care). This role has a particular focus on improving transitions in care and adherence to care and functions as a clinical advocate and educator for patients. Minimum qualifications include current unencumbered licensure with the WV Board of Registered Professional Nurses or appropriate state board, or multi-state licensure through the enhanced Nurse Licensure Compact (eNLC), and certification in Basic Life Support within 30 days of hire. Requires three years of nursing experience. Core duties include assessing and facilitating coordination of multidisciplinary care, triaging new patient referrals, collaborating with care teams, providing communication and education, symptom assessment and management, assessing health literacy, documenting patient needs and encounters in EPIC, assisting with staff and patient education, and participating in service line team meetings. Physical requirements include frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, ability to sit for extended periods, and assisting mobility impaired patients. Work environment includes standard office settings and multiple clinic offices and testing departments.
