Healthcare United is seeking a Board-Certified or Board-Eligible Hospice and Palliative Care Physician to join a community-based healthcare organization in Connecticut. This role includes a mix of inpatient, outpatient, and home-based palliative care along with hospital-based coverage. The position offers a collaborative, interdisciplinary environment with strong clinical support.
- Provide hospice and palliative care across inpatient, outpatient, and home-based settings
- Conduct goals-of-care discussions and advance care planning
- Manage pain and complex symptoms including dyspnea, nausea, fatigue, and delirium
- Participate in hospital coverage including admissions and daily rounding
- Collaborate with interdisciplinary teams including nursing, social work, and case management
- Support transitions of care between hospital, hospice, and community settings
- Participate in shared call coverage
- Maintain accurate and timely EMR documentation
- Engage in quality improvement and program development initiatives
Requirements
- Board Certified or Board Eligible in Hospice and Palliative Medicine, Internal Medicine, or Family Medicine
- Eligible for Connecticut medical licensure
- Active or eligible DEA registration
- Strong clinical and communication skills
- Ability to work effectively within a multidisciplinary team
