Healthcare United is seeking a Board-Certified or Board-Eligible Hospice and Palliative Care Physician to join a community-based healthcare organization in Connecticut. This role includes a mix of inpatient, outpatient, and home-based palliative care along with hospital-based coverage. The position offers a collaborative, interdisciplinary environment with strong clinical support.

  • Provide hospice and palliative care across inpatient, outpatient, and home-based settings
  • Conduct goals-of-care discussions and advance care planning
  • Manage pain and complex symptoms including dyspnea, nausea, fatigue, and delirium
  • Participate in hospital coverage including admissions and daily rounding
  • Collaborate with interdisciplinary teams including nursing, social work, and case management
  • Support transitions of care between hospital, hospice, and community settings
  • Participate in shared call coverage
  • Maintain accurate and timely EMR documentation
  • Engage in quality improvement and program development initiatives


Requirements

  • Board Certified or Board Eligible in Hospice and Palliative Medicine, Internal Medicine, or Family Medicine
  • Eligible for Connecticut medical licensure
  • Active or eligible DEA registration
  • Strong clinical and communication skills
  • Ability to work effectively within a multidisciplinary team
  • Commitment to patient-centered care
