The manager of home care services is responsible for the clinical, operational, and regulatory oversight of home care and hospice services.  This role ensures the delivery of high-quality, patient-centered care while supporting staff development, compliance, interdisciplinary collaboration, and organizational goals.  The manager provides day-to-day supervision of clinical staff, promotes regulatory readiness, monitors quality outcomes, and works collaboratively with leadership to ensure efficient operations and exceptional patient care.   Responsibilities   Clinical and Operational Oversight Oversees the daily operations of home care and hospice services Supervises and supports the home care and hospice nursing and interdisciplinary staff  Assists with staffing coordination and scheduling oversight Provides clinical guidance and support for complex patient situations Coordinate with intake staff to ensure acceptance of appropriate referrals and timely admissions Availability to do field visits as needed to support patient care Participates in Administrator on Call rotation to support staff after hours and on weekends Regulatory and Compliance Ensures compliance with all federal, state, Medicare, Medicaid, and accreditation standards Maintains survey readiness and assist with preparation for regulatory reviews and audits Oversees adherence to agency policies, documentation standard, and clinical best practices Monitors and address incident reports and risk management concerns Quality and Performance Improvement Provide oversight of Quality Assurance and Performance Improvement (QAPI) activities Work with Clinical Education and Outcomes Specialist to identify opportunities for process improvement and staff education Staff Leadership and Development Provide oversight of new staff orientation Conduct annual performance evaluations Promote staff engagement, accountability, and professional growth Assist with recruitment, interviewing, and retention efforts Interdisciplinary Collaboration Collaborate with physicians, facilities, community partners, and interdisciplinary team members to coordinate patient care Participate in leadership meetings, staff meetings, and weekly hospice IDT meetings Support communication to ensure continuity of care and operational efficiency   Qualifications  Qualifications Current Registered Nurse (RN) in Connecticut Bachelor’s degree in nursing Minimum 3-5 years of nursing experience in home health or hospice Knowledge of Medicare COPs, hospice regulations, home care standards Strong organizations, communication, and problem-solving skills Strong critical thinking and clinical judgement   Disclaimer The above statements are intended to describe the general nature and level of work being performed by people assigned to this classification.
