In this role, you will identify residents’ psychosocial, mental and emotional needs, connect them with appropriate services, and help residents and families navigate admissions, care planning, transitions and discharge decisions.

Responsibilities

What You’ll Do as a Social Worker:

  • Manage a social work caseload for residents on an assigned nursing unit

  • Welcome new residents and help them adjust to the long-term care environment

  • Identify residents’ psychosocial, mental and emotional needs

  • Participate in developing and reviewing individualized plans of care

  • Complete required screenings for new admissions and applicable discharge planning

  • Support residents and families through admissions, care transitions, transfers and discharges

  • Participate in decisions involving changes in care setting or level of care

  • Document resident information accurately and in accordance with facility and department standards

  • Collaborate with nursing, rehabilitation and other departments to address residents’ physical, emotional and social needs

  • Provide individual, family or group support within the scope of the position

  • Participate in staff education and in-service programs as requested

Qualifications

What You’ll Need as a Social Worker:

  • Bachelor’s degree in social work from an accredited institution; or a bachelor’s degree in a related human-services field, including human rehabilitation counseling, psychology, sociology, gerontology or special education

  • At least one year of supervised social work experience in a healthcare setting working directly with individuals

  • Casework experience

  • Experience supporting admissions or discharges is strongly preferred

  • Valid driver’s license and a clean driving record

  • Ability to work effectively with older adults, families and interdisciplinary team members

  • Strong interpersonal, communication, organization and documentation skills

  • Ability to read, write, speak and understand English at an intermediate or higher level to support safe, effective workplace communication

  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
