OCES supports older adults and individuals with disabilities by providing vital information and coordination of services. By promoting healthy, safe living for our consumers, we hope to allow them to stay active and engaged in their lives and in their own communities for as long as possible.

The HC Meals Care Manager, as part of our Community/Nutrition Program, evaluates need for home delivered meal services by our consumers and periodically reviews and reassesses service needs.

What you will do:

  • Make home visits to assess needs.
  • Develop appropriate service plans; coordinate and monitor services rendered
  • Advocate for consumers in obtaining benefits and services, and assist as appropriate
  • Maintain documentation
  • Initiate and maintain communication with appropriate community resources to better coordinate delivery of consumer services as needed
  • Conduct visits to meal sites; collaborate with meal site and other OCES staff

What you will bring:

  • Bachelor's degree in Social Work or related field
  • Current social work license preferred
  • Excellent verbal and written communication skills.
  • Excellent interpersonal and conflict resolution skills.
  • Excellent organizational skills and attention to detail.
  • Strong analytical and problem-solving skills.
  • Proficient with Microsoft Office Suite and other software
