We're looking for an experienced RN Case Manager to join our Home Health team on the West Side of Cleveland, offering top-quartile pay, a $5,000 sign-on bonus, and the tools and support to do your best work every day.
Compensation
- $39.00 – $46.00 per hour / productivity point, depending on experience
- $5,000 sign-on bonus
- Full-time / 40 hours per week

What You'll Do
- Conduct comprehensive patient assessments and develop individualized, evidence-based care plans tailored to each patient's needs
- Coordinate services across an interdisciplinary team to ensure all aspects of care are efficiently managed
- Perform home visits, monitor patient progress, and adjust care plans as needed
- Educate patients and families on health management, medications, and available resources
- Serve as the primary point of contact for patients, families, and referring providers
- Maintain accurate and timely documentation in the EMR, including OASIS assessments, within required timeframes
- Ensure compliance with Medicare, Medicaid, and agency regulations
- Participate in team meetings, case conferences, and ongoing professional development

Requirements
Must-Have:
- Active RN license (Ohio) + current BLS certification
- Associate or Bachelor's degree in Nursing (BSN preferred)
- Minimum 2 years of clinical nursing experience, including at least 1 year in a home health setting
- Reliable vehicle and valid driver's license with ability to travel throughout the West Side Cleveland service area
- Familiarity with Medicare and Medicaid regulations
- Proficiency in electronic medical record (EMR) systems and home health-specific software
Preferred / Nice-to-Have:
- OASIS certification or demonstrated OASIS experience
- Familiarity with HCHB or similar home health EMR
- Strong caseload management and time management skills
- Experience with care coordination across interdisciplinary teams
