We're looking for an experienced RN Case Manager to join our Home Health team on the West Side of Cleveland, offering top-quartile pay, a $5,000 sign-on bonus, and the tools and support to do your best work every day.

Compensation

  • $39.00 – $46.00 per hour / productivity point, depending on experience
  • $5,000 sign-on bonus
  • Full-time / 40 hours per week

What You'll Do

  • Conduct comprehensive patient assessments and develop individualized, evidence-based care plans tailored to each patient's needs
  • Coordinate services across an interdisciplinary team to ensure all aspects of care are efficiently managed
  • Perform home visits, monitor patient progress, and adjust care plans as needed
  • Educate patients and families on health management, medications, and available resources
  • Serve as the primary point of contact for patients, families, and referring providers
  • Maintain accurate and timely documentation in the EMR, including OASIS assessments, within required timeframes
  • Ensure compliance with Medicare, Medicaid, and agency regulations
  • Participate in team meetings, case conferences, and ongoing professional development

Requirements

Must-Have:

  • Active RN license (Ohio) + current BLS certification
  • Associate or Bachelor's degree in Nursing (BSN preferred)
  • Minimum 2 years of clinical nursing experience, including at least 1 year in a home health setting
  • Reliable vehicle and valid driver's license with ability to travel throughout the West Side Cleveland service area
  • Familiarity with Medicare and Medicaid regulations
  • Proficiency in electronic medical record (EMR) systems and home health-specific software

Preferred / Nice-to-Have:

  • OASIS certification or demonstrated OASIS experience
  • Familiarity with HCHB or similar home health EMR
  • Strong caseload management and time management skills
  • Experience with care coordination across interdisciplinary teams
