HOME HEALTH AIDE

JOB SUMMARY:

A paraprofessional who is specifically trained, competent and performs assigned functions of personal care to the patient in their residence under the direction, instruction and supervision of the registered nurse (RN).

QUALIFICATIONS:

1. Certification by the Colorado Department of Regulatory Agencies as a Certified Nursing Assistant or ability to obtain certification within four months of starting employment.
2. Have a sympathetic attitude toward the care of the sick and elderly.
3. Ability to carry out directions, read and write.
4. Ability to effectively communicate to demonstrate competency in the provision of care and services safely and effectively and treat patients with dignity and respect to person and property.
5. Maturity and ability to deal effectively with the demands of the job.
6. 18 years of age.
7. High School Diploma or GED.

LINE OF AUTHORITY:    Reports to the Clinical Manager/Nursing Supervisor/RN/ PT/OT/ST

RESPONSIBILITIES:

1. Provides services that are ordered in the plan of care and that the aide is permitted to perform under State law.
2. Performs hands-on personal care.
3. Performs simple procedures as an extension of the therapy or nursing services as assigned.
4. Assistance in ambulation or exercises as assigned.
5. Assists with medications that are ordinarily self-administered as assigned.
6. Understands and adheres to established Agency policies and procedures.
7. Completes appropriate visit records in a timely manner as per Agency policy.
8. Reports changes in the patient's condition and needs to the appropriate personnel.
9. Performs household services essential to health care in the home as assigned.
10. Attends in-service and continuing education programs as scheduled and necessary.
11. Attends staff meetings and patient care conferences as scheduled.
12. Follows Agency policy for cleaning equipment between patient uses.
13. Carries out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews.
14. Completes all work assigned by the RN.
15. Does not accept assignments for a patient with special needs for which he/she has not received appropriate training.
16. Observes confidentiality and safeguards all patient related information.
17. Reports any variance, accident or unusual occurrence to the appropriate personnel.
18. Participates in QAPI activities as requested.
19. Maintains acceptable attendance status, per Agency policy.
20. Reports incomplete work assignments to RN.
21. Appearance is always within Agency standard; is clean and well groomed.
22. Demonstrates effective time management skills through daily documentation and infrequent overtime for routine assignments.
23. Attends position related in-services. Attends all mandatory in-service programs as scheduled; minimally 12 hours/year.
24. Maintains clean and neat work environment.
25. Demonstrates sound judgment and decision making.
26. Maintains current CPR certification.
27. Performs other duties as assigned.
    WORKING ENVIRONMENT:
     Works indoors in Agency office and patient homes and travels to/from patient homes.
