Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments

Position Requirements:
- Physical Therapist: Current, 1+ years - Required
- Recent/Current Home Health PT experience - Required
- Must have valid Driver’s License and Personal Auto Insurance
Education, License and Certification Requirements:
- Degree in Physical Therapy - RequiredÂ
- PT License for State applying or Compact if applicable -Â Required
- BLS Certification from AHA - Required
Job Description:
Primary Function/General Purpose of Position Â
The Physical Therapist is responsible for providing prescribed treatments of physical therapy which involve a complex and sophisticated level of modalities designed to restore the patient to his/her maximum rehabilitative potential. The Physical Therapist evaluates plans and provides care to patients with a deficit in self-care, mobility, safety, range of motion or strength. The Physical Therapist is responsible for supervising and directing the care provided by the Physical Therapy Assistant. The Physical Therapist may supervise the care provided by the aide to increase the patient’s functional abilities and rehabilitative potential.Â
Essential Job Functions Â
- Provides direct hands-on patient care following patient’s plan of care and current standards of practice for Physical Therapy.Â
- Documents all care provided, coordination of services, orders, goals, etc. in the patient’s electronic medical record in an accurate and timely manner.Â
- Supervises care provided by the PTA and Home Health Aides to ensure that treatment goals are being met.Â
- Coordinates with other disciplines, agencies/companies, families, etc. to ensure that the care needs of the patient are met.Â
