  • Performs initial and ongoing accurate and comprehensive assessments of each patient.
  • Accurately completes OASIS data sets.
  • Obtains necessary physician/LIP orders and collaborates with the physician/LIP as the patient's needs dictate.
  • Develops and implements a written plan of care for each patient in coordination with the physician/LIP and other healthcare team members to facilitate optimal patient outcomes.
  • Guarantees that changes in patients’ clinical status are communicated to other healthcare team members, necessary office staff, and physician(s)/LIP(s) as per Company policy.
  • Educates patients, their families/caregivers, and other staff as appropriate.
  • Provides skilled nursing care to patients appropriate to the patient’s clinical status and needs and in accordance with the standard of care for patient’s age.
  • Provides clear, concise documentation in medical record as it relates to reimbursement guidelines and policy and procedure.
  • Appropriately assesses and reassesses pain. Utilizes appropriate pain management techniques and educates the patient and family regarding pain management.
  • Performs aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Participates in case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
  • In coordination with the Interdisciplinary team, plans patient discharge from the Agency.
  • Recommends for needed referrals to other disciplines and/or community resources.
  • Obtains needed supplies and equipment to provide care.
  • Demonstrates knowledge of medications and their correct administration based on age of patient and patient's clinical status including the seven medication rights.
  • Supports cultural practices as long as these practices do not harm the patient/family or interfere with the planned course of medical treatment.
  • Provides supervision and mentorship to LPN/LVN and Home Health Aides and completes supervisory visits as required. Guarantees co-signatures in accordance with state regulations.
  • Follows up with obtaining lab results and notifies the physician/LIP of significant changes.
  • Participates in the agency Quality Assurance Performance Improvement (QAPI) program.
  • Provides back office/workflow support as needed.
Qualifications
  • Graduate of an accredited nursing program.
  • Current licensed Registered Nurse (RN) in the state of practice.
  • Experience as an RN with one of the following preferred:
    • One year of experience in an acute care setting within the last 24 months;
    • Six months of home health experience within the last 12 months.
  • Credentials in specialty areas preferred (e.g. wound care, OASIS certification, IV, ET).
  • Valid driver’s license, current automobile insurance, and reliable automobile.
  • Current CPR.
