Job DetailsJob Location: Bryan , OH 43506Position Type: Full TimePosition:  Outpatient Case Manager      Location: Bryan and Wauseon  Classification:  Full Time, Hourly, Non-Exempt GENERAL SUMMARY: The Outpatient Case Manager position is a valuable part of the outpatient treatment team. The outpatient treatment team consists of the site supervisor, therapists, case managers, nurses, and psychiatrists. This team actively works together and collaborates regarding the care of outpatient clients seeking treatment for mental health, substance use, and dual diagnosis disorders. This is an office-based position, but will require some work in the clients’ community setting. The goal of this team is to work in collaboration with the clients who are managing substance use disorders and/or dual diagnosis disorders and who are engaged in outpatient services. PRINCIPAL DUTIES AND RESPONSIBILITIES: Works to develop rapport and trusting relationships with clients served.  Assists client in learning about substance use disorder and/or dual diagnosis disorder symptoms. Works with client to problem solve barriers to care. Connects clients to resources in the community, such as doctor appointments, social security, Medicaid, therapy appointments, self-help meetings, etc. that assist client in remaining stable and managing illness. Facilitates mental health and/or substance use disorder groups. Collaborates and consults with treatment team regarding treatment of client. Manages caseload and maintains clinical documentation within the client record and billing for services. Follows agency policies and procedures and maintains compliance with accreditation bodies – CARF and OMHAS. MINIMUM QUALIFICATIONS: High School diploma required, Associates Degree in Social Work or helping related field or Bachelor Degree in Social Work preferred.  Chemical Dependency Counselor Assistant (CDCA) license required. Licensed Social Worker preferred. Training and experience in the mental health and addiction field required. Valid driver’s license, proof of insurance, and ability to be insured by agency provider. Must be 21 years of age. Subject to BCI background check, pre-employment drug screen, and reference checks.
