Nurse Practitioner-Home Visits

Base Salary: $105,000-$120,000

General Physician, P.C. is recruiting for a Nurse Practitioner or Physician Assistant to join our Home visits Service Line to provide patient care in Buffalo, NY conducting home visits . In this position you will deliver evidenced based care for Primary Care patients in coordination with the Primary Care team. This is a flexible and autonomous position that will allow you to provide a full spectrum of comprehensive medical services to patients that are partners with General Physician, PC.  

 This is a full-time position, scheduled Monday through Friday in the Buffalo area.  

 Requirements:   

• Graduate of an accredited NP or PA program;   
• Prior inpatient, high acuity experience preferred as a RN or APP.
• Board certification  
• New York State license   
• DEA registration   
• BLS, ACLS and Infection Control certifications.   
• A clinical background in geriatric, adult or family medicine is preferred.  
• Shared on call with other providers
