Buffalo Center for Nursing and Rehabilitation is looking for a Wound Care Registered Nurse (RN) for our Skilled Nursing Facility located in Buffalo, NY.
Position Summary:
Responsible for planning, organization, coordination and execution of certain aspects of the assigned facilities' Wound Care program as detailed below in accordance with current Federal, State and Local guidelines and regulations, accepted standards of practice, facilities policies, and as may be directed by Administration and/or Director of Nursing.

- Reports to Regional Clinical Director
The wound care coordinator wound care responsibilities for assigned facilities will include, but are not limited to:
- Conduct weekly wound rounds with wound care consultant(s) / provider and conduct MD and family notifications (including documenting such notifications), document comprehensive wound notes, and follow-up documentation (care plan updates, order entry, etc.).
- Complete weekly skin monitoring for all facility residents and document it in PCC.
- Complete weekly wound assessment documentation in PCC.
- Review new admissions and readmissions for accurate wound information (within a reasonable time frame)
- Review new or worsening wounds.
- Reviews pressure ulcer prevention interventions.
- Makes recommendations relating pressure relieving devices.
- Update care plans.
- Review treatment orders / implementation/ intervention.
- Complete audits (as applicable).
- Coordinate wound care with interdisciplinary team (dietitian, rehab. services, social service, nursing) etc.
- The wound care coordinator is not responsible for initial new admission/readmission skin assessments.
- ADON/designee will verify the skin assessment on admission (within 24 hours) and add as applicable - to the wound care provider list
- The wound care coordinator will be responsible to review the initial admission skin assessment and documentation within a reasonable time frame
- The ADON is still Responsible for the overall Wound care program - management of wound tracker, initial skin assessment of all new admits and readmits, reviewing newly identified wounds or other skin alteration immediately upon notification and adding such to the wound provider list and wound tracker when applicable.
