Overview:

We are seeking a skilled and compassionate Pharmacist to join our team. As a Pharmacist you will play a crucial role in providing medication counseling in support of professional medical staff.

Location:462 Grider Street Buffalo, NY 
   Department: Pharmacy 
   Shift: Per-Diem - Day Shift

Pay Range: $67,000.00-$75,000.00 Annually

Essential Duties of a Pharmacist: 

1. Completes medication reconciliation and counseling for all initial patients
2. Provides patients with symptom and medication counseling on health-related problems as directed by the attending physician
3. Takes and records or reviews and interprets the patient's medical and social history
4. Screens patients to assess need for immediate care or referral to other health personnel
5. Good knowledge of clinical medicine, medical science
6. Ability to make preventive and therapeutic medication recommendations based on accurate medical diagnosis
7. Ability to relate well to supervising physicians
8. Ability to communicate well with and relate well to colleagues and patients
9. Physically capable of performing the essential functions of the position with or without reasonable accommodation
10. Conducts follow-up phone calls with high-risk patients
11. Documents in patient charts and maintains detailed records of patient treatment
12. Performs related duties as assigned by the medical staff
13. Deliver exemplary customer service in order to provide a positive patient experience across the organization
14. This position may require meeting with patients in community settings based on clinical needs

Minimum Qualifications of Pharmacist:

Education:

Doctor of pharmacy (Pharm D.) degree from an accredited school

Experience:

1-2 years of experience in a clinical pharmacy, preferred

License/Registration/Certification:

Basic Life Support (BLS) Certification, required

Possession of a valid, active license and current registration to practice as a Pharmacist in New York State as issued by the New York State Education Department

Specialized Knowledge/Skills:

• Proficiency in Electronic Medical Records (EMR), computer skills 
• Strong organizational and time management skills with the ability to manage multiple schedules and prioritize tasks
• Excellent interpersonal skills to interact with patients, surgeons, and other healthcare team members
• Strong understanding with insurance verification and authorization process
