Rehabilitation Admission Coordinator

Working within a team environment, the Rehab Admissions Coordinator evaluates patients and makes decisions for Inpatient Rehabilitation admission as appropriate. This team member communicates with patients, families, physicians, acute care staff, Inpatient Rehabilitation staff, other team members and insurance representatives as necessary throughout the process. They maintain collaborative relationship with physiatry consult service, Inpatient Rehabilitation staff and other team members. Additionally, they educate patients, families, health care providers, insurance representatives and community about Rehabilitation Center Programs.

Education:

• State of Vermont Registered Nurse (RN) license required.
• CRRN preferred.

Experience:

• 3-5 years work in rehabilitation program or with patients who need rehabilitation.
