Responsibilities:
路 Plans, coordinates, implements, and evaluates the nursing care of all patients.
路 Utilizes clinical knowledge and skills to identify patient needs and coordinate services and referrals to meet these needs
路 Works closely with the inter disciplinary team to provide a continuous appropriate, comprehensive, and responsive plan of care for patients.
Requirements
路 Must be at least 21 years of age.
路 A valid non-restricted RN license in the state of Colorado is required.
路 Must have a valid non-restricted driver鈥檚 license.
路 Flexibility and availability to work a weekend schedule if needed.
路 At least 1 year of home health or hospice experience preferred.
