Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. What you’ll do as an Occupational Therapist OT:

  • Evaluate and assess individual patient needs, creating personalized treatment plans to inspire functional independence.
  • Utilize evidence-based methods and therapeutic tools to improve motor functions, cognitive abilities, and daily activities.
  • Collaborate with a multidisciplinary team (physical therapists, speech therapists, doctors, nursing staff) to provide comprehensive care.
  • Educate and empower patients and their families, providing them with the knowledge and techniques to continue improving at home.
  • Monitor patient progress, documenting outcomes and adjusting therapy plans for optimal recovery.
