Job Summary
Evaluates and assesses patients to establish functional, measurable goals to promote restoration of functional abilities. Provides treatment in accordance with a written plan of care to meet established goals. Plans for discharge and communicates findings and progress according to professional licensure/certification, scope of practice, and departmental/discipline policies and procedures. Produces other reports as needed. May supervise clinical students and support staff, including certified/licensed assistants, technicians, and volunteers. Provides a caring relationship that facilitates health, and healing. Â Performs other duties as assigned.
Job Responsibilities
Evaluates the patient using subjective and objective information.
•   Develops a written plan of care
•   Provides treatment according to established goals on the plan of care.
•   Evaluates the effectiveness of the plan of care. Reassess the patient's functional status.
•   Provides discharge planning information which includes patient/family education, follow-up, and referral to community resources.
•   Contributes to the development of staff and students.
•   Participates in activities designed to improve health care delivery.
•   Completes assigned goals.
Education Minimum Required
Skill in communicating clearly and effectively using standard English in written, oral, and verbal formats to achieve high productivity and efficiency. Skill to write legibly and record information accurately as necessary to perform job duties.
Special Skills Minimum Required
Computer literacy
Basic skills.
BLS certification within 14 days of hire date
