What You'll Do
As a Home Health Physical Therapist, you'll help patients regain mobility, confidence, and independence through personalized, one-on-one care in their homes.
Conduct physical therapy evaluations and OASIS assessments
Develop and implement individualized care plans to restore function and mobility
Deliver hands-on therapeutic interventions and home exercise programs
Educate patients and caregivers on proper exercise, movement, and safety techniques
Collaborate with nurses, physicians, and other care team members to optimize outcomes
Maintain accurate and timely clinical documentation
ā What You'll Bring
Active New Jersey Physical Therapist (PT) license
Master's or Doctorate in Physical Therapy
Home care experience (required)
CPR certification, valid driver's license, reliable transportation, and insurance
Excellent communication and organizational skills
Passion for providing personalized, in-home rehabilitative care
