Short Description

  • Coordinate and facilitate field-based outreach, patient engagement, and care navigation activities as a member of the Mobile Integrated Health (MIH) team, working alongside an MIH Clinician to identify, engage, and support individuals living with substance use disorder throughout the City of Camden.
  • Accompany the MIH Clinician in an MIH vehicle to provide non-emergent responses to overdose-related calls and conduct community-based outreach, patient follow-up, and care coordination in homes, shelters, public spaces, healthcare settings, and other community locations.
  • Engage individuals through multiple pathways to care, including overdose responses, hospital discharges, community referrals, self-referrals, peer referrals, and proactive outreach efforts.
  • Conduct outreach and re-engagement activities with individuals who have disengaged from treatment or been lost to follow-up.
  • Assess barriers to health and recovery, including housing, transportation, food security, healthcare access, and other social determinants of health.
  • Provide education on substance use disorder, medications for opioid use disorder (MOUD), harm reduction, recovery supports, and community resources.
  • Facilitate referrals to treatment providers, behavioral health services, harm reduction programs, recovery supports, healthcare providers, and social service agencies.
  • Maintain current knowledge of treatment, recovery, healthcare, housing, transportation, and social service resources.
  • Document patient encounters, outreach activities, referrals, and outcomes in the electronic health record and other required data systems.
  • Track and report outreach activities, patient engagement, referrals, and program outcomes to support grant reporting, evaluation, and quality improvement efforts.
  • Work a flexible schedule, including evenings and weekends as needed.

Experience Required

3 - 5 years experience in community based project planning, public health programming, case management, working with grant funded programs, or related work.

Ā 

Ability to manage a variety of stakeholders, provide direct patient services, conduct community outreach, and deliver education to EMS personnel and/or Skilled Nursing Facility personnel

Ā 

Experience in community outreach/education programs.Ā 

Education Requirements

Bachelor Degree in Public Health, Social Work, or related field required;

Ā 

Master’s Degree in Public Health, Social Work, or related field preferredĀ 

Special Requirements

Excellent communication skills, both written and verbal. Ā 

Ā 

Computer literate, comfortable with Microsoft Office software (Outlook, Excel, Powerpoint, Word).Ā 

Ā 

Electronic Health Record experience helpful

Ā 

Demonstrated experience in patient-facing roles and community outreach, with the ability to engage diverse populations and foster trust through effective communication and culturally competent service delivery.
