Essential Functions Skills and expertise in current evaluation and remediation techniques. Evaluates patients, develops effective treatment plans and obtains orders from the referring physician. Communicates well with supervisor and other health team members regarding patient progress, problems, and plans. Records evaluations, daily treatment notes, weekly progress notes, and discharge summaries per policy and procedure. Complies with the Physical Therapy Act in the State. Supervises Physical Therapy Assistants and/or Physical Therapy Aides and RNA’s in direct resident care and patient related activities. Flexible Full-Time hours as agreed upon by the Director of Rehabilitation and direct supervisor.   B. Major Duties and Responsibilities Participates in patient care conferences and Rehabilitation Conferences.   Participates in in-service training program for other staff in the facility. Records treatment charges per policy and procedure. Instructs patient’s families or nursing staff in maintenance program. Participates in discharge planning. Secure necessary durable medical equipment for residents to facilitate independence in mobility (i.e. braces, walkers, canes, elevated toilet seats, and grab bars). Assists with monthly wheelchair clinic. Reports any problem with department equipment so that it is maintained in good working order. Acts as a clinical preceptor for affiliating Physical Therapy or Therapy Assistant Schools. Performs other duties as assigned. QualificationsC. EDUCATION AND KNOWLEDGE Degree from an accredited program in Physical Therapy. Current California State License in Physical Therapy. Sufficient verbal and written communication skills to perform the tasks required. D. Physical Requirements: (With or without the aid of Mech. Devices) Must be able to move intermittently throughout the workday. Must be able to cope with mental and emotional stress on the job. Must be in good general health and demonstrate emotional stability. Subject to frequent interruptions. Must be able to relate to and work with the ill, disabled, children, emotionally upset, and at times hostile people within the facility. Must be able to lift, push, pull and move a minimum of 50 lbs. Be able to work with patients, families, visitors, personnel and government agencies under all circumstances. Must be able to assist in the evacuation of patients during an emergency situation. May be requested to work overtime. Subject to potential falls, odors, burns from equipment, and hazardous materials. Also, to potential exposure to infectious waste, diseases, and conditions including TB, AIDS, and Hepatitis viruses. E. Work Environment While performing the duties of this job, the employee occasionally works near moving mechanical parts and is occasionally exposed to toxic or caustic chemicals, risk of radiation and vibration. The noise level in the work environment is usually moderate. The employee must be able to work near other co-employees and concentrate efforts on tasks despite noise, interruptions and general commotion.
