Description

The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y. 


Jordan Health is now seeking a LPN Lead who, under the guidance of the Practice Manager, is primarily responsible for providing high quality patient care and oversees both the clinical operations and administrative coordination in order to ensure a steady workflow, uninterrupted service and maximization of patient satisfaction in a family medicine setting.

Requirements

The LPN Lead will ensure that:

· Jordan Health patients receive high quality, consistent, competent, patient-centered care. 

· Jordan Health maintains a specific focus to support Team Base Care. 

· Jordan Health administrative activities are completed timely and accurately. 

· Jordan Health resource staffing activities ensure coverage and proper training for key positions to minimize vacancies.

· Jordan Health Family Medicine financial and regulatory objectives are supported. 

· Jordan Health strategic objectives and key programs are supported and developed. 

 

Education And Experience Required:

· Accredited LPN Program. 

· 2+ years of healthcare experience in an ambulatory setting. 

· Previous supervisory experience or team leadership preferred. 

· Strong interpersonal, and collaborative skills; ability to foster and maintain sound relationships with key stakeholders influencing without authority. 


Licenses And Certifications Required:

· Current NYS LPN License without any negative actions. 

· Certification to administer medications. 

· Current provider BLS/CPR through the American Heart Association.

· Infection Control Certificate. 

· Valid NYS Driving License and reliable transportation. 


Special Skills, Knowledge Required: 

· Excellent computer skills, fluent with Microsoft Office (Word, Excel, Power Point)

· Familiarity with Electronic Health Records preferred.
