• Current, valid Pennsylvania Registered Nurse (RN) license
• Home health nursing experience strongly preferred
• OASIS experience required
• Strong knowledge of OASIS assessment requirements and Medicare regulations
• Excellent attention to detail and organizational skills
• Strong written and verbal communication skills
• Ability to work independently while collaborating effectively with clinical staff
• Proficiency with electronic health records and Microsoft Office

• Reviewing OASIS assessments for accuracy, completeness, and consistency
• Ensuring OASIS documentation supports the patient’s clinical condition and plan of care
• Reviewing documentation for compliance with Medicare and home health regulations
• Identifying discrepancies, missing information, and opportunities for improvement
• Collaborating with clinicians to clarify and correct OASIS documentation when needed
• Providing guidance and education to clinical staff regarding OASIS requirements and documentation standards
• Reviewing assessments for appropriate coding and clinical consistency
• Supporting accurate patient outcomes and reimbursement through thorough OASIS review
• Staying current with changes to OASIS guidance, Medicare requirements, and home health regulations
• Maintaining timely and accurate documentation within the electronic medical record
• Working collaboratively with the clinical team to promote quality, compliance, and positive patient outcomes
