Job DetailsJob Location: Bedrock Recovery Center - Canton, MA 02021Position Type: Full TimeSalary Range: $52,500.00 - $57,000.00 Salary/yearOur mission. Northeast Recovery is a network of Drug and Alcohol Addiction Treatment Facilities that provide a variety of levels of care to those who are prepared to begin their journey toward recovery. We are looking for a full time Aftercare Coordinator at our Bedrock Recovery Center location in Canton, MA! Schedule: Full Time Salary: $52,500 – $57,000 annually (depending on experience and qualifications) Position Summary The Aftercare Coordinator works collaboratively with the clinical team to develop and implement discharge and aftercare plans for patients receiving treatment for mental health and substance use disorders. This position helps ensure patients are connected to outpatient providers, recovery supports, housing resources, and other services prior to discharge. Key Responsibilities Develop individualized aftercare and discharge plans for patients preparing to leave treatment. Coordinate referrals to outpatient treatment programs, therapists, psychiatrists, recovery programs, and community resources. Schedule follow-up appointments prior to discharge to support continuity of care. Collaborate with the multidisciplinary treatment team to ensure appropriate transition planning. Maintain strong relationships with community providers and referral partners. Assist patients with accessing housing resources, transportation, and recovery supports. Document discharge plans and referrals in the electronic health record in accordance with regulatory standards. Maintain patient confidentiality in compliance with HIPAA and 42 CFR Part 2. Qualifications Bachelor’s degree in Social Work, Psychology, Human Services, or related field preferred. Experience in behavioral health, mental health services, substance use disorder treatment, or case management preferred. Experience working with co-occurring mental health and substance use disorders strongly preferred. Strong knowledge of community resources and discharge planning processes. Excellent communication, organization, and coordination skills. Preferred Credentials LADC, or related behavioral health licensure Recovery Coach or Peer Specialist certification Why Work for Us!
