Job Summary
We are looking for a Direct Support Professional to join our team! You will directly work with individuals, following a one-on-one care plan in their home. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.Â
ResponsibilitiesÂ
- Assist the client with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
- Care for client by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
- Maintain confidential records of client care, condition, progress, or problems to report and discuss observations with the supervisor as per HIPPA Guidelines
- Build relationships with individuals by conversing or reading aloud to help keep them mentally healthy and alertÂ
- Give medicationsÂ
- Engage patients in exercises or other activities Â
Qualifications
- Graduated High School and any other healthcare certification in CNA., HHA program
- High School Diploma or GEDÂ
- One year prior professional experienceÂ
- Driver’s license requiredÂ
- CPR certification requiredÂ
- Job offer is contingent on passing a drug screen and federal background check Â
