Position Summary

The Licensed Social Worker is responsible for supporting the emotional, behavioral, educational, and social well-being of children residing in our residential programs. Reporting to the Director of Social Work, this position develops comprehensive psychosocial assessments, collaborates with interdisciplinary teams, and advocates for each child's individual needs. The successful candidate will demonstrate professionalism, compassion, and a commitment to Second Family's mission, core values, policies, and all applicable federal, state, and local regulations.

Job Duties

  • Conduct comprehensive psychosocial assessments for each child.
  • Develop and maintain detailed psychosocial histories, including:
    • Family and social background
    • Medical and behavioral history
    • Placement history
    • Services received while at Second Family, Inc.
    • Discharge planning and future care recommendations
  • Develop, coordinate, implement, and monitor Individual Plans (IPs), Individual Service Plans (ISPs), and Treatment Plans to ensure individualized, goal-oriented care.
  • Coordinate, facilitate, and lead Individual Plan (IP)/Treatment Plan meetings involving parents or guardians, referring agency representatives, attorneys, REM Case Managers, CASA representatives, and other members of the interdisciplinary team as appropriate.
  • Participate in the development and review of Individualized Education Programs (IEPs).
  • Provide comprehensive case management and advocacy services for assigned children and youth.
  • Ensure the emotional, social, behavioral, and psychosocial support needs of medically fragile and/or developmentally delayed children and youth are appropriately assessed, addressed, and monitored.
  • Support the medical care of assigned individuals by coordinating and obtaining required documentation for physician appointments, specialty consultations, surgeries, medical procedures, therapy services, and other healthcare needs.
  • Coordinate with physicians, nurses, therapists, educators, guardians, parents, and community agencies to ensure continuity of care and successful implementation of treatment plans.
  • Attend and actively participate in interdisciplinary treatment team meetings.
  • Represent Second Family, Inc. during:
    • School meetings (when the Education Coordinator is unavailable)
    • Home visits
    • Court hearings and judicial reviews
    • Community appointments and interviews
  • Develop, implement, and monitor behavior support plans for assigned individuals, as needed, in collaboration with the interdisciplinary team.
  • Maintain accurate, timely, and confidential documentation in accordance with agency policies, COMAR regulations, licensing standards, Medicaid requirements, and accreditation standards.
  • Travel between Second Family residential locations, hospitals, schools, courts, and community agencies as required.

 Requirements  

  • Current Maryland Social Work license (LMSW, LCSW, or LCSW-C as applicable).
  • Minimum of one (1) year of supervised social work experience in a healthcare, residential, behavioral health, or related setting working directly with individuals.
  • Current American Heart Association CPR and First Aid certification (or ability to obtain upon hire).
  • Valid driver's license with reliable transportation and the ability to travel between program locations.
