POSITION OVERVIEW:

This position serves as a member of the Illinois Crisis Prevention Network’s (ICPN) Support Service Teams responsible for providing individual therapeutic supports to children and adults with an intellectual/developmental disability, many of whom are dually diagnosed with a mental illness.

 

Salary: $56,000-$76,000

NETWORK OR DEPARTMENT:

Illinois Crisis Prevention Network

 

ESSENTIAL FUNCTIONS:

 

  • Provides case management services to people and their families by identifying and linking necessary resources.
  • Develops an Action Plan and/or Behavior Plan when necessary for people served.
  • May deliver individual ABA therapy services when appropriate for people served.
  • Works closely with other services providers including Individual Service Coordination agencies, agencies providing DHS-DDD waiver funded services, families, and schools to collaborate and facilitate consistent service delivery across all systems.
  • Develops and facilitates comprehensive trainings for families and service providers specific to person receiving support.
  • Complete and maintain all documentation and weekly reporting for clinical chart.  This includes but is not limited to action plans, weekly updates, training materials, and billing
  • Participates in weekly team meetings, didactics, and case consultations.

 

SKILLS AND ABILITIES:

  • Knowledge of resources and clinical supports for people with intellectual/developmental disabilities and mental illness. 
  • Able to make independent decisions and problem solve.
  • Able to deal effectively with staff, program participants, family members, visitors, government agencies/personnel, and the general public
  • Able to promote good staff relations and employee morale
  • Able to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, necessary for providing quality care
  • Able to execute policies, procedures, and regulations and when necessary, explain them to program participants, family members, members of the community, and staff from other agencies
  • Excellent oral and written communication skills
  • Excellent interpersonal skills
  • Strongly self-motivated

 

PHYSICAL DEMANDS:

  • Lift over 40 lbs from floor to waist unassisted.
  • Push/pull up to 75 lbs and maneuver wheelchairs weighing up to 200 lbs.
  • Operate motor vehicles.
  • Ability to remain seated or standing for extended periods.
  • Frequent activities include driving, lifting, kneeling, crouching, reaching, and repetitive motion.
  • Ability to move quickly to prevent harm to self when working with a person who is aggressive
  • Be able to push wheelchairs up to 200 pounds

 

QUALIFICATIONS:

  • Must possess a bachelors or master’s degree in behavior analysis and has attained or is working toward independent BCAB certification.
  • Previous experience working with children and/or adults with intellectual/developmental disabilities and mental illness.
  • Valid Illinois driver’s license with a good driving record
