This role involves providing direct patient care and education to assigned patients to ensure optimal healing and recovery. Key Responsibilities: - Assess the needs of patients with basic and complex wounds to determine appropriate care strategies. - Develop a comprehensive wound management plan tailored to individual patient needs and assist staff in its implementation. - Document the wound care plan and any revisions made to ensure continuity of care and accurate patient records. - Coordinate the evaluation of wound care, monitoring, and documenting patients’ responses to treatment. - Obtain and document pictures of wounds upon admission and weekly to track progress and inform treatment adjustments.QualificationsRequired Education: - Bachelor’s degree or a combination of an associate’s degree in an approved Registered Nurse program with meaningful and appropriate experience. Required Experience: - Experience in assessing, planning, implementing, and evaluating wound care. - Experience in providing direct patient care and education to assigned patients. Preferred Experience: - Two (2) years of experience in an acute or LTACH (Long-Term Acute Care Hospital) setting is preferred. Required Skills and Abilities: - Current Registered Nurse license in the state where practicing is required. - Current Basic Life Support (BLS) certification. - Advanced Cardiovascular Life Support (ACLS) certification within 6 months of hire. - Proficiency in electronic medical records and documentation.  Preferred Skills and Abilities: - Certified Wound Ostomy Nurse (CWON) certification.
