### Core Responsibilities

• Assess & Treat: Evaluate adult patients' mobility, strength, and functionality in a home-based setting; design and execute custom rehabilitation plans.
• Educate: Guide patients and families on home exercise routines, body mechanics, and injury prevention.
• Collaborate & Document: Maintain accurate clinical documentation and coordinate with an interdisciplinary healthcare team to optimize outcomes.

### Qualifications

• Education & License: Degree in Physical Therapy from an accredited program; valid Arizona PT license (or license-eligible).
• Skills: Strong clinical assessment skills, excellent communication, and a patient-centered approach.
• Physical: Ability to lift/assist patients and perform physical tasks inherent to mobility therapy.
