Key Responsibilities:

  • Conduct home visits to assess client needs, ensure adherence to care plans, and provide training or oversight for caregivers.
  • Complete and submit accurate documentation for each client visit in a timely manner.
  • Communicate with clients and caregivers to address service-related concerns, changes in condition, or care quality issues.
  • Respond promptly to service interruptions, urgent needs, or client concerns while in the field.
  • Collaborate with PAS leadership to maintain care standards and regulatory compliance.
  • Monitor and report on caregiver performance, client satisfaction, and any changes in service needs.


Job Qualifications:

  • May have completed two (2) years of full-time study at an accredited college or university; OR may substitute one year of full-time employment in a supervisory capacity in a health care setting for each required year of college.
  • Reliable transportation and ability to travel locally for home visits.
  • Excellent communication and organizational skills.
  • Proficient in Microsoft Office and comfortable using electronic documentation systems.
  • Bilingual (English/Spanish) REQUIRED.
