ABOUT THE ROLE:

Following the patient-centered medical home health care delivery model, the Registered Dietician participates in a patient-focused, team-based approach to promote health and wellness with patients.  As part of the medical care team, the services provided by the Registered Dietitian aim to improve patient health outcomes and knowledge pertaining to nutrition. Primary responsibilities include medical nutrition therapy, nutrition education and nutrition counseling. Serves as a specialist consultant for internal operations of Community Health.

 

FUNCTIONS OF THE POSITION: 

  • Implement the Nutrition Care Process as defined by the Academy of Nutrition and Dietetics’ Standards of Care (screening, assessment, diagnosis, intervention, monitoring/evaluation, documentation, outcomes management)
  • Engage patients in the goal setting process and utilize Motivational Interviewing and other techniques to assist in goal achievement; develop nutrition care plans that are individualized as well as culturally and clinically appropriate
  • Works in collaboration with Care Managers and Community Health Workers in the reduction of barriers to care and addresses social determinants of health to provide the highest level of nutritional counseling.

  • Plans, direct, and provides nutritional diabetes care to patients; providing nutritional classes as needed.
  • Serves as a nutritional resource for patients requiring chronic disease management.  
  • Attends the ADCES Core Concepts course and strives to achieve CDCES certification within at least 3 years from date of hire
  • Actively participates in coordination and collaboration in our Population health program.
  • Collaborates and communicates with other members of the health care team of each patient 
  • Provide appropriate evidence/research-based patient education materials when applicable, taking into consideration the patients’ learning style and health literacy
  • Provide 1:1 and group medical nutrition therapy, nutrition education and nutrition counseling in-person or virtually
  • Performs nutrition assessment and intervention on patients based on professional standards
  • Possesses the knowledge to research insurance reimbursement guidelines to determine adequate coverage for nutritional coverage
  • Assists with planning, implementing and evaluation of nutritional tools, dietary guidance education resources and all other pertinent nutritional information to support Community health
  • Instructs patients and families with nutritional guidance, dietary modification and implementational strategies
  • Works with interdisciplinary team and delivers nutritional recommendations
  • Documents nutrition interventions, recommendations, and patient education per Community health policy.
  • Participates in Care Conferences and works collaboratively with our Diabetes Educators and care Management team.
  • Independently anticipates and always responds appropriately to the age-related patients’ psychosocial and physical needs. Independently obtains and integrates clinical information in terms of age and developmental needs of patients and responds appropriately.


SKILLS REQUIRED FOR SUCCESS:

  • Bachelor’s degree in nutrition or health related field
  • Registered Dietician (RD) by Commission on Dietic registration
  • Current state licensure if applicable
