Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications, treatments, and skilled nursing procedures in a home health setting
- Monitor and document patient progress, vital signs, and changes in condition
- Coordinate care with physicians, therapists, and other healthcare team members
- Educate patients and family members on diagnoses, medications, and home care techniques
- Ensure compliance with all state, federal, and agency regulations and standards of care
- Provide specialized nursing interventions as needed
Requirements:
- Active Registered Nurse (RN) license in the state of Maryland
- Previous home health, hospice, or community nursing experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Reliable transportation and valid driver's license
- CPR/BLS certification required
- Compassionate, self-motivated, and able to work independently
