Overview:

Referral Specialist for Monmouth and Ocean County. The Referral Specialist will provide referrals and resources to connect women and families to health care and other social service providers in the community. Responsibilities for this position include community outreach in both Monmouth and Ocean Counties coordinating with outreach to locate women/families without telephones to achieve contact for an initial assessment and medical care preconception.

Salary: $40,000

Responsibilities:

  • Document all aspects of client interaction in customized computer database.
  • Complete follow-up calls to clients as assigned.
  • Develop and maintain knowledge of health care and public health services, community resources, and relevant policy issues.
  • General office duties as assigned.
  • Provide excellent customer service in every interaction.
  • Engage in ongoing professional development activities

Education Requirements: 

High School diploma, Associate’s degree (preferred) and a minimum 1 year experience in maternal child health field and/or health related field. At least 1 year’s experience in community based public health field; excellent communication and problem solving and public speaking skills. Must demonstrate initiative, creativity, work well independently and as part of a team. This position requires travel in Monmouth and Ocean County.

Work Experience Requirement:

Minimum of one to two years’ experience working with high risk, underserved racially and ethnically diverse communities, either in the human services, social service and/or maternal child health agency. Must demonstrate ability to work cooperatively with the staff and a variety of persons external to the family support center, including representatives of other agencies and parents. Must have experience working effectively in a multi-cultural setting and the ability to model and teach appropriate interaction with children and parents. . Experience working in the area of women’s health is a plus. 

Requirements:

  • Bilingual English/Spanish (Written and Spoken)
  • Experience with computer systems, data entry and word processing
  • Knowledge of community resources in Monmouth and Ocean Counties
  • Excellent written and verbal communication skills
  • Ability to handle complex calls and situations
  • Highly resourceful team-player, with the ability to also be extremely effective independently
  • Proven ability to handle confidential information with discretion, be adaptable to various competing demands, and demonstrate the highest level of service and response.
  • Must possess knowledge of NJ programs that serve Families
