We are seeking a Care Coordinator to help guide patients through their treatment journey from admission through discharge. This position plays a key role in making sure care is coordinated, barriers are addressed, and patients are connected with the resources and support they need to successfully continue their recovery.

What You’ll Do

As a Care Coordinator, you will support patients throughout the treatment process and serve as an important connection between the patient, treatment team, outside providers, and community resources.

Key responsibilities include:

  • Support the admission and intake process, including patient orientation, required documentation, consents, and releases.
  • Assist with initial screenings and assessments within the scope of the position.
  • Identify patient needs, strengths, barriers, and available supports.
  • Coordinate care across clinical, medical, nursing, peer support, behavioral health, and community providers.
  • Participate in interdisciplinary treatment team meetings and communicate updates regarding patient needs and progress.
  • Assist with treatment planning and ongoing review of patient goals.
  • Help patients identify practical and achievable recovery goals.
  • Coordinate referrals to healthcare providers, outpatient treatment, recovery housing, peer support, and community-based resources.
  • Help patients access resources related to housing, transportation, employment, insurance, benefits, legal needs, and other barriers to recovery.
  • Begin discharge planning early in the treatment process.
  • Coordinate follow-up appointments and continuing-care resources prior to discharge.
  • Communicate with outside providers and referral partners in accordance with applicable releases and confidentiality requirements.
  • Maintain accurate, complete, and timely documentation in the electronic medical record.
  • Support utilization review and authorization processes as assigned.
  • Recognize behavioral, emotional, or safety concerns and promptly communicate concerns to the appropriate clinical or medical team member.
  • Maintain professional boundaries with patients at all times.
  • Protect patient confidentiality in accordance with HIPAA, 42 CFR Part 2, and organizational policy.
  • Participate in required meetings, training, supervision, and quality-improvement activities.
  • Perform other duties as assigned to support patient care and facility operations.

Requirements

  • Bachelor’s degree in social work, psychology, human services, behavioral health, or a related field required.
  • Minimum of two years of experience in behavioral health, substance use disorder treatment, healthcare, case management, or a related human-services setting preferred.
  • Experience with intake coordination, case management, care coordination, or discharge planning preferred.
  • Experience working in a substance use disorder treatment setting strongly preferred.
  • Knowledge of behavioral health, substance use disorders, recovery principles, and trauma-informed care.
  • Familiarity with ASAM criteria preferred.
  • Behavioral health certification or licensure such as CADC, IADC, LBSW, LMSW, or another relevant credential preferred.
  • Strong written and verbal communication skills.
  • Strong organizational, documentation, and problem-solving skills.
  • Ability to maintain confidentiality and professional boundaries.
  • Current CPR and First Aid certification or ability to obtain within required onboarding timelines.
  • Ability to successfully complete all required pre-employment screenings, training, and competency requirements.

Work Conditions & Physical Requirements

This position works in a residential behavioral health treatment environment and involves regular interaction with individuals who may be experiencing emotional distress, behavioral health symptoms, substance use cravings, or other challenges associated with early recovery.

The position may require:

  • Frequent sitting, standing, and walking.
  • Regular computer and telephone use.
  • Frequent verbal and written communication.
  • Occasional lifting of up to 25 pounds.
  • Responding appropriately to behavioral or patient-safety concerns.
  • Adherence to infection-control and workplace-safety requirements.
  • Occasional evening or weekend work based on program needs.
