Medication Therapy Management Pharmacist

Location: Hybrid - with strong preference for candidates reside within commuting distance to Cedar Rapids regions

 

We're seeking a MTM Pharmacist to join our team! In this role, you'll be a member of the clinic-based care team, the MTM Pharmacist proactively and collaboratively identifies and coordinates the resolution of medication-related issues based on an interdisciplinary, patient-specific care plan.  Working at the top of their license and taking a comprehensive approach, the pharmacist helps patients achieve their best possible outcomes from their medication therapies. The pharmacist also serves as the medication expert to raise the overall knowledge of the care team and improve workflows where possible.

Responsibilities

What You'll Do:

  • Perform Comprehensive Medication Reviews for high risk patients.
  • Adjust doses when allowed by Collaborative Practice Agreement, or provide recommendations to prescribers.
  • Perform medication reconciliation to ensure patient’s medication list is as accurate as possible, particularly during transitions of care.
  • Order appropriate lab tests to ensure adequate monitoring of medication therapies.
  • Provide recommendations to help patients manage their out of pocket costs for medications.
  • Provide patient education around medications, devices, and disease states.
  • Quality Improvement Participate in projects to improve the medication use process at the clinic and health system in general.

  • Provide formal and informal training to other staff to improve their medication-related knowledge.

  • Assist in developing and revising standards of care for the MTM Pharmacy team.

  • Collaborate with clinic staff to improve workflows and address site-specific needs.

  • Participate in appropriate meetings within the health system.

Qualifications

Education:

Required: PharmD

 

Experience:

Preferred: 3-5 years’ experience as a practicing pharmacist or pharmacy residency

 

License(s)/Certification(s):

Required:

  • Valid driver’s license when driving any vehicle for work-related reasons
  • Current license to practice pharmacy in Iowa.
