POSITION: CARE NAVIGATOR

LOCATION: BUTLER, PA

WAGE: $20 PER HOUR

Adagio Health is seeking an empathetic, proactive Care Navigator to connect patients, clients, and community members directly to the care they deserve. In this role, you will be the bridge between clinical services and community support-addressing social determinants of health, providing warm handoffs, and guiding individuals through health insurance, nutrition, and preventive healthcare programs.

WHAT YOU WILL DO

 

  • Connect & Guide: Assess client and patient needs related to health, wellness, and social determinants of health (SDOH). Identify barriers to care (such as transportation, food security, or cost) and connect individuals with internal programs and trusted external community partners.

  • Provide Warm Handoffs: Serve as a dedicated point of contact, ensuring seamless transitions between clinical appointments, nutrition support, and community services.

  • Expand Healthcare Access: Serve as a certified Pennie Assister to help community members navigate and enroll in health insurance. Partner closely with WIC programs to support enrollment, outreach, and intake.

  • Community Outreach: Build and maintain active, positive relationships with local service organizations, grassroots groups, and community partners to increase awareness and accessibility of services.

  • Client Education: Deliver practical health education and share vetted resources to empower individuals to make informed healthcare decisions.

  • Cross-Department Collaboration: Partner with teams across Clinical Services, Disease Prevention, and Nutrition & Education to coordinate holistic, person-centered care.

  • Care Coordination & Documentation: Maintain accurate, timely documentation of all patient interactions, referrals, and outcomes in data management systems to support ongoing care quality and reporting

WHAT YOU BRING

  • Accredited degree in Social Work, Public Health, or a related field.

  • Active Community Health Worker (CHW) certification.

  • Excellent interpersonal, verbal, and written communication skills with the ability to build trust across diverse populations and clinical teams.

  • Must be comfortable and proficient working with Microsoft Office products (Word, Excel, Outlook, Teams) and database systems.

  • Experience in case management, care coordination, or care planning.
  • Background in customer service or direct community-facing support.

  • Experience navigating and assisting with enrollment processes for medical insurance products (e.g., Pennie, Medicaid, CHIP).

  • Ability to complete regional travel as needed for community outreach and partner site visits.

  • Navy / Coast Guard: Hospital Corpsman (HM), Health Services Technician (HS), or Community / Public Affairs

  • Air Force / Space Force: Aerospace Medical Service (4N0X1), Public Health (4E0X1), Health Services Management (4A0X1), or Family Support personnel

  • Any Branch: Experience in casualty assistance, unit medical logistics, family readiness groups (FRG), transition assistance, or community liaison roles
