Charleroi, PA Location Overall Function: Performs a variety of tasks and functions required by the agency for successful operations and expeditious delivery of quality services. Essential Functions: Complete initial referrals for Caregiver Support and Dom Care programs. Receives, processes, and completes all documentation of Caregiver Support Program invoices, including but not limited to uploading file attachments and enters service deliveries. Prepares and submits all required program forms, documentation, and correspondence in an accurate and timely manner to ensure Caregiver Support Program compliance. Prepares, maintains, and tracks requirements for Dom Care Providers and Consumers including but not limited to Provider Certifications, Rental Agreements, Rent Rebates, insurance information, etc. Responsible for troubleshooting and correcting discrepancies on program reports. Assist Care Managers with completing required telephone contacts with consumers. Contributes to and maintains current records as well as documentation in Social Assistance Management System (SAMS) for all case activities. Assist Care Managers with other program related tasks as necessary. Adheres to all policies, laws, regulations and codes of ethics and confidentiality as outlined by federal and state laws, and agency policies and procedures. Qualifications Associate Degree in Human Services or Business Management from an accredited college and one year of Aging experience preferred; any relevan combination of skills, experiences and training may be considered at the discretion of the agency. Demonstrated knowledge of Department of Aging Program regulations and policies and other appropriate government regulations. Must not be excluded from participation in Medicare, Medicaid, or any other federal health care program. Must successfully obtain Criminal History Clearance from PA State Police and Child Abuse Clearance from the PA Dept. of Human Services, and FBI Fingerprint Clearances. Demonstrated ability to organize, coordinate and manage multiple responsibilities. Demonstrated knowledge of financial entitlement programs and available social service programs The ability to establish and maintain effective working relationships with the public.
