Physical Requirements:

The employee is regularly required to speak and hear sufficiently to communicate effectively by phone or in person, at normal volumes. Qualifications:

Bachelors Degree in Nutrition or related field required. Must have R.D. registration and be eligible for West Virginia State license. Certification as a Diabetes Educator is preferred.

Basic Function:

Collaborates with providers to identify and educate patients in need of nutrition counseling. Works in an environment requiring a high degree of independence and program coordination.

Duties and Responsibilities:

  1. Collaborates with providers to identify and connect with patients in need of nutrition counseling.
  2. Establishes treatment plans and communicates with patients regarding program compliance.
  3. Meets directly with patients to identify educational needs; provide individual instruction, utilize informational handouts/booklets, or refer to community resources as needed.
  4. Identifies ways to improve care of patients, and makes recommendations to the Medical Director and Quality Assurance Committee.
  5. Participates in various patient program committees.
  6. Performs research and recommends ways to expand and improve nutrition programs.
  7. Participates in quality improvement of the program in conjunction with other services offered.
  8. Other duties as assigned.
