Telehealth Presenter

Job Summary:

Personalized Health Partners is seeking dedicated Telehealth Presenters to support our Never Alone program within a Skilled Nursing and Rehabilitation setting. This individual will play a key role in ensuring residents receive timely access to their healthcare providers through high-quality telehealth support. The ideal candidate is patient-centered, organized, and comfortable working in a clinical environment.

Key Responsibilities:

  • Facilitate and manage resident telehealth visits and scheduling, ensuring timely access to providers.
  • Prepare residents for virtual appointments, including setting up equipment and ensuring privacy.
  • Communicate professionally with residents, staff, and providers to ensure a smooth telehealth experience.
  • Assist with documentation, appointment coordination, and follow-up tasks as needed.
  • Support clinical teams by escalating urgent issues and helping residents communicate their health concerns effectively.
  • Maintain telehealth equipment and troubleshoot basic technical issues.
  • Demonstrate excellent time management to coordinate multiple daily appointments.
  • Uphold professionalism, compassion, and customer service in all interactions.

Qualifications:

  • Preferred: Background as a CNA, MA or LPN.
  • Preferred: Experience in a Skilled Nursing Facility (SNF) or rehabilitation setting.
  • Strong customer service skills and ability to communicate clearly with residents and staff.
  • Knowledge of clinical environments and workflows.
  • Excellent time management, organizational skills, and reliability.
  • Comfortable with technology and able to learn new systems quickly.
