About The Position

This opportunity begins on June 2, 2025, and spans 60 days with potential for extension. The position involves working Monday through Friday, 8:00 AM to 4:30 PM. As the sole Nurse Practitioner, you will be providing comprehensive primary care services, focusing mainly on chronic care and medication management, with occasional urgent care cases.


Job Details: 

Location: West Virginia 

Facility Type: FQHC (Multi-specialty clinic)

Start Date: June 2, 2025 

LOA: 60 days 

Provider Type Needed: Family Practice Nurse Practitioner 

Schedule: Monday-Friday, 8:00 AM - 4:30 PM 


Responsibilities and Duties: 

• Provide comprehensive primary care services to patients of all ages 

• Manage chronic conditions and perform medication management 

• Handle occasional urgent care cases (minimal suturing required) 

• Conduct patient examinations, diagnose conditions, and develop treatment plans 

• Order and interpret diagnostic tests 

• Prescribe medications and provide patient education 

• Collaborate with other healthcare professionals to ensure optimal patient care

• Maintain accurate and detailed medical records 

• Adhere to FQHC policies and procedures 

• Participate in quality improvement initiatives


Additional Information: 

• West Virginia Nurse Practitioner license required for this position 

• Experience in managing chronic conditions is essential 

• Comfort with working independently as the sole NP is crucial 

• Minimal urgent care or suturing skills required (current providers only sutured 3 times in 8 years) 

• Opportunity for assignment extension may be available
