JOB TITLE: Program Manager – Supported Living 

DEPARTMENT: Community Living 

SUPERVISOR'S TITLE: Assistant Director - Supported Living  

POSITIONS SUPERVISED:  Direct Support Professionals – Supported Living and Home Leaders – Supported Living      

JOB STATUS: Full-Time, Non-Exempt

SCHEDULE: Monday–Friday, 8:00am–4:30pm, plus 24/7 on-call 

 

Summary:

Provide leadership and oversight for Supported Living services, ensuring high-quality, person-centered care for individuals served. This role is responsible for staff supervision, service delivery, and program operations, while ensuring compliance with agency standards, regulatory requirements, and best practices.


Key Responsibilities:


Staff Leadership & Supervision
• Supervise, coach, and support Direct Support Professionals (DSPs) and Home Leaders
• Assist in staff recruitment, hiring, onboarding, training, and performance management
• Provide ongoing feedback, conduct evaluations, and address performance concerns
• Ensure adequate staffing coverage and collaborate with Home Leaders to fill open shifts

Service Delivery & Program Oversight
• Oversee implementation and effectiveness of Individual Support Plans (ISPs)
• Monitor client progress and adjust service strategies in collaboration with the interdisciplinary team
• Ensure services are person-centered and promote independence and self-advocacy
• Maintain regular communication with individuals served, families, and providers
• Conduct weekly home visits to monitor service quality, staff performance, and compliance
• Provide or coordinate transportation and ensure access to community resources

Scheduling & Operational Management
• Review and approve staff schedules to ensure appropriate coverage and compliance
• Maintain oversight of staffing patterns and operational needs across assigned homes
• Participate in on-call rotation and provide 24/7 support as required
• Work flexible hours, including direct service shifts during staffing needs or crises

Documentation & Administrative Duties
• Ensure accurate and timely completion of all required documentation
• Complete administrative tasks such as billing, timesheets, and mileage reports

Qualifications & Skills:

• Strong leadership, supervision, and team development skills
• Excellent communication (written, verbal, and electronic)
• Ability to manage multiple priorities in a fast-paced environment
• Strong problem-solving and decision-making abilities
• Demonstrated empathy and commitment to person-centered services

Physical & Work Requirements:

• Ability to work a flexible schedule, including evenings, weekends, and on-call responsibilities
• Ability to work in multiple home settings as needed
• Ability to provide direct care support when required (including personal care tasks)
• Ability to lift, push, or pull up to 40 lbs
• Frequent standing, walking, and travel between service locations

• Valid driver’s license, reliable transportation, and proof of insurance

Certifications & Training Requirements:

• CPR, First Aid, and Bloodborne Pathogens certification  

• Certified Restraint Training

• Core A & B certification 

• Completion of required agency and regulatory trainings (e.g., Human Rights, Safety, Medication Administration)
