Job DetailsJob Location: Center City - Charlotte, NC 28202Position Type: Full TimeEducation Level: High SchoolSalary Range: $15.00 - $16.00 HourlyTravel Percentage: FrequentJob Shift: 7:30am to 4:30pmJob Category: Direct CareIf making a lasting difference in someone's life is a priority for you and your career goals, we want to help you get there. LIFESPAN is hiring compassionate, hardworking, friendly, and reliable Community Guides to assist participants and planning teams in developing social networks and connections within their local communities.     Schedule: Monday through Friday 8:00 AM to 4:00 PM/ 9:00 AM to 5:00 PM   What we do:  LIFESPAN’s mission is to empower children and adults with disabilities by providing education, employment and enrichment opportunities to live, work and play in their communities. We focus on the interests of each individual we support to ensure their well-being, dignity and right of choice. As a Community Guide, YOU are the MVP of our organization by providing resources to identify and eliminate barriers to personalized, person centered care. In return, LIFESPAN is committed to helping you succeed by providing a supportive team environment, training, growth and development, and career advancement. Join our team of passionate and caring professionals to make a meaningful difference in people's lives.     What you’ll get:  Competitive pay  Flexible schedules including full and part time opportunities   Paid training  Full time employees have access to benefits including paid time off; medical, dental, vision, and life insurance; short- and long-term disability insurance; 403b retirement plan with employer match; and perks galore!     What you’ll do:  Responsible for Informing and coordinating community resources including coordination among primary, preventative, and chronic care providers.  Assist in locating and accessing non-Medicaid community supports and resources that are related to achieving Individual Support Plans (ISP) goals.  Assist in locating options for renting or purchasing a personal residence, assisting with purchasing furnishings for the personal residence.  Provide instruction and counseling which guides the participant in problem solving and decision making.  Advocate on behalf of the participant.  Support the person in preparing, participating in and implementing the Individual Support Plan (ISP).  Provide training on the Individual and Family Directed Supports Option, if the participant is considering directing services and supports.  Assist management with guidance of the individual budget.  Assist with coordinating of services with the Financial Supports Services provider, if the participant is self-directing services under the Employer of Record Model, including guidance on use of the individual and family directed budget (self-directed budget).  Provide information on recruiting, hiring, managing, training, evaluating, and changing support staff, if the participant is self-directing services.  Assist with the development of schedules and outlining staff duties, if the participant is self-directing services.  Assist with understanding staff qualifications, documentation and record keeping requirements, if the participant is self-directing services.  Provide on-going information to assure that participants and their families/representatives understand the responsibilities involved with self-direction, including reporting on expenditures and other relevant information and training.  Coordinate services with the Agency with Choice if the participant is directing services under the Agency with Choice Model.  Be available to work flexible hours including nights, weekends and overtime as required.  Depending on the person you support, you may be required to:  Administer medications. Training is provided. Nursing team supports you.  Provide transportation to and from designated sites.  Assist people with mobility needs, e.g. utilize transfer techniques, a Hoyer lift and/or gait belt. Training is provided.  QualificationsWhat you need:   Must be at least 18 years of age   Possess a valid driver’s license for a minimum of two consecutive years  Pass criminal record and healthcare registry check   Pass driving record check if providing transportation   High school graduate or equivalent (GED)   Commitment to achieve competency in required knowledge, skills, and abilities   Note: Current First Aid/CPR blended or classroom certification is required. If you do not currently have certification from an approved provider, our American Red Cross certified instructors will provide you the training.  Ability to lift up to 40 pounds independently (per policy)  LIFESPAN is committed to a culture that inspires longevity, latitude, creativity, balance, and fulfillment. LIFESPAN is an equal opportunity employer.
