Hiring: Licensed Practical Nurse (LPN)
 Position PurposeÂ
The primary role of a Licensed Practical Nurse (LPN) is to provide comprehensive oversight, coordination, and hands-on care in addressing all health-related matters affecting program participants. This includes both direct and indirect health concerns, ensuring the physical and emotional well-being of individuals through proactive assessment, timely intervention, and ongoing collaboration with interdisciplinary team members, families, and external healthcare providers. The LPN plays a critical role in maintaining a safe, responsive, and supportive environment that promotes optimal health outcomes for all individuals involved in the program.Â

 Major ResponsibilitiesÂ
- Maintains nursing documentation and medication records Â
- Assists residents in learning about their medications; assures that medications are taken as physician ordered and recorded in electronic MAR Â
- Provides nursing care including, but not limited to, wound care, trach care, diabetic counseling, etcÂ
- Assesses residents’ health and emotional needs Â
- Communicates with other members of the nursing and program staff; communicates serious issues to Nurse Manager and Program Director Â
- Maintains confidentiality, including keeping voice low and not talking about residents in public areas •Other duties as assigned

Job Qualifications and CompetenciesÂ
Job Qualifications Â
- Current DE RN/LPN licensureÂ
- Current CPR/First Aid certificationÂ
- Valid Driver’s LicenseÂ
- Act 33/34 clearancesÂ
- FBI clearance as neededÂ
- All trainings required by licensureÂ
- Must complete a Physical, TB test, and Hep B titer. Â
