Registered Nurse (RN) – Home Health
Pay: $45–$49/hour
Location: Gastonia, NC
Schedule:
Monday–Friday, 8:00 AM – 5:00 PM
No weekends or on-call required
Holiday coverage may be required if it falls on a weekday

Position Overview:
The Registered Nurse (RN) will provide comprehensive nursing care to individuals in a home health setting. This role involves assessing patient health, implementing and monitoring care plans, coordinating with multidisciplinary teams, and ensuring quality and compliance with care standards. The RN will promote health maintenance, disease prevention, and may provide supervision to other nursing staff as needed.
Key Responsibilities:
Observe, assess, plan, implement, and evaluate nursing care for patients with acute, chronic, or rehabilitative needs in home settings.
Collaborate with physicians and healthcare professionals to plan and evaluate programs addressing patient health needs.
Coordinate communication and care actions across various disciplines to ensure patient safety and continuity of care.
Manage medication processes, including ordering, reviewing, monitoring, discontinuation, and disposal in compliance with regulations.
Maintain and review policies for medication administration, storage, and error prevention; ensure annual updates as needed.
Notify physicians of changes in patient condition and ensure timely updates to care plans.
Accurately transcribe physician orders and maintain appropriate medical documentation.
Complete required nursing documentation daily, monthly, quarterly, and annually as applicable.
Document health status and nursing observations in patient notes regularly.
Maintain proper infection control and ensure medical supplies and equipment meet safety standards.
Supervise Licensed Practical/Vocational Nurses (LPNs/LVNs) and provide direction as needed.
Coordinate and follow up on patient appointments and ensure timely implementation of care recommendations.
Communicate effectively with inpatient and discharge coordinators during patient hospitalizations.
Provide new employee and annual training on health, safety, and patient-specific care needs.
Ensure unlicensed staff receive training related to individual patient health, safety, and disease prevention.
Qualifications:
Active RN license (compact license accepted or willingness to obtain).
Minimum of 1 year of home health experience required.
Preferred experience with:
HomeCare HomeBase (HCHB) documentation system
Wound care/wound vac management
IV therapy and medication administration
Strong organizational, communication, and clinical assessment skills.
Ability to work independently and manage multiple cases efficiently.
